Put It On The Shelf.

Follow these steps below.

  1. Fill out the following form completely.
  2. Make sure that you select the appropriate plan that you are requiring.
  3. Look over your completed form to make sure everything filled in is correct.
  4. Submit your form and allow up to a 24 hour time period for a response. Thanks!

The process after your form is submitted.

  1. If everything is filled out correctly, you will be notified via e-mail of an invoice to pay for the plan that you have selected on the form.
  2. If there are any issues with your completed form you will not receive that invoice but rather you will receive an e-mail acknowledging the issues that need to be corrected.
  3. Once you have paid the invoice your item will be added to the shelf for others to see.
